最近剛好遇到一個問題：有個病人體重299公斤(無誤)，他要使用enoxaparin....問題來了喔，大家都知道，這個藥1mg/kg q12h(對正常腎功能的病患而言)，而且沒有什麼很有效的antidote....所以，大家覺得這劑量怎麼給好？
